OK, obviously this has spoiler potential, so read at your own discretion.

A few people have mentioned being unable to keep everyone alive in the final mission of the game, so I opted to write this small guide to help out.
firstly, the more people you have loyal, the better chance you'll have of keeping everyone alive. Loyalty makes them fight harder and makes them tougher to kill. Ideally you'll want to have everyone who is possible to be loyal at this point, to be loyal. And of course, having recruited all squad members will have helped significantly, and I will write this with the assumption you have done so. You will also need to have made all possible upgrades to the Normandy.

During the course of it you will have to assign people various roles. It's very important to choose the right people. The most important thing is that you do not send anyone who is disloyal, they will die.

*For the vent infiltration, send either Tali, or Legion. Sending anybody else, with the possible exception of Kasumi, will get them killed.

*For the fire team leader, select either Garrus, Miranda or Jacob. Anybody else will get your technician killed.

*For the biotic specialist, choose Jack or Samara/Morinth. Anybody else, someone dies.

*Same characters apply for the second fire team leader.

*For the escort, send either Jack, Mordin, Kasumi or Tali. Sending anyone else will make it more difficult for your rear guard to survive later. Sending a weak defender here, who will survive if loyal, increases your chances later on.

Now, your final team. Who you pick here is vital. If you definitely want to keep everyone alive, take the weakest defenders with you. Namely, Jack, Mordin, Kasumi or Tali. Do not take strong defenders; namely, Garrus, Zaeed or Grunt. The more strong defenders you leave behind, the more chance there is of keeping someone alive. You can also guarantee the safety of a disloyal crew member by taking them with you, but if they are a strong defender I recommend leaving them behind. I took Jack and Kasumi. Follow these instructions and you should be fine.

Lastly, a little bit of battle advice for harder difficulties. When I was playing this on Insanity, every Collector was equipped with a biotic barrier, which can be a pain. This doesn't really matter on anything below Hardcore, but I recommend keeping Miranda and Thane with you up until the final battle. As they both have the Warp ability (preferably upgraded to Heavy Warp), you'll cut through the barriers with no problem at all.

Advice came in handy for my insanity playthrough, thanks. I used Jacob as fire team leader on both occasions, Tali went in the vent which is risky as her suit can rip, which means certain death but she was OK. As always I used the obvious choice of Samara as biotic shield specialist, and Mordin was the escort. Miranda and Thane tore through all the enemy barriers for me making things far less painful. There were a few occasions when they make you think someone might have died or received a fatal wound, but everyone survived on the team, even the crew made it this time round. Good times.

Edit: Also worth mentioning it's difficult when Miranda and Jack have their argument to keep both of them loyal, this time round it was Jack who was angry with me, so I was worried she would die, but thankfully she made it.

Glad it was useful. Somehow I managed to keep both of them loyal, no idea how I managed it. Only person whose loyalty I was lacking was Zaeed, because it requires really high Paragon/Renegade and I did it way too early in the game.

I've never had a problem with Jack v. Mira, but thats because I always go full paragon or full renegade. Only exception being Zaeed, who I always take the renegade route with for the reason you brought up musashi.

I wish I could remember what I did... but I do know I would do Zaeed's mission very early and have enough Paragon points to keep him loyal. I remember it being an odd conversation tree to keep him loyal, but I always managed.

If I recall correctly the Renegade route is fairly easy, if you let him kill Video. Could be that I just didn't have enough paragon points for whatever reason, but I wish I could remember how much I had at the time.

Yeah, the Renegade part was easy.... let the people in the factory burn and die while going after the asshole and letting Zaeed kill him.

If you went the Paragon route, you had to save the people in the burning factory and then do something in his conversation tree to get the Paragon option. Normally ME2 just left it as BLUE and RED options (so you knew which were paragon and renegade), but he had something different. Now I need to go youtube it.
